What is Indiana’s Medicaid Waiver Program?

Waiver is Indiana’s state and federally funded Medicaid program for individuals with intellectual or developmental disabilities. The program offers a variety of services to assist individuals in living a productive life within their home and community. These services range from providing medically necessary devices and housing needs to childcare services so parents can continue working.

Who Qualifies for Waiver?

To qualify for Waiver you must have been diagnosed with an intellectual disability, developmental disability, or related condition at or before the age of 22. You must meet a level of care determining that you have functional limitations and that Waiver services would assist you in continuing to live at home. Though Waiver services are funded through Medicaid, they are not based on financial need for those under 18.


Is There a Waitlist?

The waitlist fluctuates depending on the number of applications and the ability to process their applications. Before Covid, the waitlist for Waiver had decreased significantly as the program grew. Since Covid, the waitlist has grown and the waitlist may be up to two years.

Individuals ages 18-22 who have finished high school and/or dependents of military active duty and veterans are considered priority without being placed on a waitlist.

How Do I Apply?

Indiana now has an online portal for Medicaid Waiver applications!

You may also apply for the Waiver program by contacting your local Bureau of Developmental Disabilities Services (BDDS) office or print one online. You will need two forms:

  1. Application for Developmental Disabilities Services (state form 55068)

  2. Confirmation of Diagnosis (state form 54727)
